Our endodontist and team are dedicated to providing you with the highest quality care available. Our office does not contract with any insurance company due to the treatment limitations they impose, regardless of diagnosis. Our commitment is solely to you, our patient, not to any insurance provider.

Your insurance benefits are a benefit negotiated and purchased by your employer and offered to you. The contract exists between you, your insurance company and your employer. We are not party to this agreement and do not have specific details about your benefits.

As a courtesy, we will assist you in filing electronic claims to help you receive the maximum benefits you are entitled to. However, since we cannot guarantee payment or the exact amount, we kindly request that all patients arrange financial details prior to their appointment.

We offer various financial arrangements for your convenience, including:

  • Payments made in full by cash or check will receive a 5% courtesy adjustment.
  • Payments in full can also be made using major credit cards.
  • A 60% down payment allows us to file your insurance and await ‘assignment of benefits’. We require a signed ‘Signature on File’ authorization form to apply any remaining balances to your credit card. A form will be provided to you.
  • If external financing is used, the entire fee will be applied, and your insurance reimbursement will be mailed directly to you.
  • Extended payment plans are available through outside lending sources, including CareCredit®, which offers plans up to 6 months with no interest.*
